ASSISTED LIVING CARE
In a home like setting, we offer the perfect solution for seniors who need some assistance with daily activities. Residents can live independently in their own suites while receiving the services they need from our well-trained and attentive staff.
MCI CARE PROGRAM
Our Mild Cognitive Impairment Care Program provides a personalized and innovative approach to the care of seniors with mild cognitive disorders and other related memory impairments. Although it’s a non-locked facility, we’ve taken extensive measures to ensure a secure and stimulating environment—in order to safely address the specific needs of each resident.
SHORT-TERM STAYS
Short-term stays are an excellent choice for seniors needing a little extra help after a hospitalization or during an illness.
HOSPICE CARE
By partnering with the area’s finest provider’s, in our home like setting, we can provide high quality palliative care and maintain the highest possible quality of life.
OUR SIGNATURE WELLNESS PROGRAM
Includes a personalized care assessment to determine assistance needed with Activities of Daily Living [ADL]. This may include simple reminders, supervision, or physical/cueing, assistance with activities of either daily living: bathing /showering, dressing, toileting, eating/feeding, ambulation, transferring, incontinent care. Behavior Management to include Minimum/Moderate or Extensive. Weekly housekeeping, linens and personal laundry are also included, as needed.
OUR eMEDICATION MANAGEMENT PROGRAM
Our Signature Wellness Program also include Greenfield’s eMedication Management Program. This unique program includes electronic ordering services from our corporate pharmacy partner, supervising distribution and administering of medication through Greenfield’s electric medication management software. Over-sight for any Resident that self-administer their own medication (ensure that resident continues to administer their own medication, taking medication per Physician orders, and prescribed medications match current physician orders.
MONTHLY COMMUNITY EVENTS AND ACTIVITIES
At Greenfield, we recognize that keeping our residents active and engaged is essential to the health and well-being of the people in our care.
Our daily, weekly and monthly activities program is both extensive and comprehensive. It covers a wide range of activities—from cooking and physical exercise, to arts and crafts.
In addition to our regular activity program, we also arrange numerous special events—for the entertainment of our residents. These events include things like concerts, outings and guest speakers.
